Our 1st Meeting

Prior to our 1st visit, please download the Intake Form and fill it out completely.  Answer each question as thoroughly as possible and know that there is "NO" wrong answers.  This information helps to give us a "Starting Point" for your future sessions and your life as a whole.  Additionally, you will "get-to-know" yourself better before the session by taking the time to reflect on your past experiences.  Moving forward is possible, but it helps greatly to know where you have been.  Thank you for your cooperation.  Additionally, I have provided other often requested forms that you can read or pass along to family, friends, co-workers, etc.  They may serve as a vehicle of HOPE for those who have lost their way

The fee for each 50-minute counseling session is as follows:

  Initial Visit   $80.00
Additional Visits $70.00

                                                

A sliding scale is available if criteria are met based on income.

Please make checks payable to GCS (Georgia Counseling Services, LLC). All credit and debit cards are accepted.  In an effort to keep costs down, GCS does NOT file insurance for counseling sessions.  However, each session will provide a receipt for you to submit to your respective insurance agency.  Our desire is to keep sessions affordable and available to all who need help.  All accounts past due over 90 days will be charged a 20 % APR on the outstanding balance and may be reported to a collection agency.

Cancellations must be given 24 hours in advance or a $70.00 fee will be charged.  If you call less than 24 hours in advance to cancel your appointment and the appointment cannot be filled, there will be a $70.00 charge.
